Description:

As an trainee ACP, you will be expected to work alongside the medical and nursing teams to promote the delivery of a high standard of patient centred care within a supported environment. The post is full time position that requires flexibility in working, with rotation to weekends/Bank holidays as part of an agreed rota with other members of the ACP team. The post holder will utilise an evidence-based approach to the management of acute patients and will be expected to provide care and make clinical decisions in accordance with agreed clinical pathways of care and proficiency of own skill set. This will also include history taking, examination and investigation skills to timely and safely progress the patients journey.
Multi-disciplinary collaboration and working will be expected using communication skills to enhance both the service and its sustainable delivery. Acute Medical Unit (AMU) The role involves working on AMU supporting the acute physicians undertaking the acute medical ward round ensuring any investigations are reported on and acted upon. The post holder will work towards taking a role in the advanced clinical assessment and treatment of a diverse and undetermined caseload of patients, who may have highly complex and/or chronic presentation. They will determine clinical diagnosis and treatments and maintain records as an autonomous practitioner.
Same Day Emergency Care (SDEC) The SDEC led by the ACP team and is open between the hours of 8am to 10pm. This unit receives direct referrals from the GP and patients are streamed from the Emergency Department. The post holder will perform a assessment of patients presenting in Same Day Emergency Care (SDEC) setting.As the trainee ACP you would be expected with supervision to take a clinical history, clinical examination, formulate a differential diagnosis and treatment plan. The post holder would be expected to guide and advise the nursing team on patients presenting to the department in the absence of a experienced ACP.
The post holder will be expected to support the nursing team with difficult clinical skills. The post holder will be supported contribute to the education and training of the multidisciplinary team and will engage in clinical supervision, self-reflection, research/audit and quality initiatives that contribute to the improvement of care for adults presenting to MCHFT
