Description:

We have a really exciting opportunity for you to join our Care of the Elderly Advanced Clinical Practitioner team at University Hospitals Coventry & Warwickshire NHS Trust as a Trainee Advanced Clinical Practitioner in Care of the Elderly and Acute Frailty Service (Band 7). The successful post holder will be supported to undertake the MSc in Advanced Clinical Practice and upon successful completion will become a Band 8a.
We are looking for a dynamic, inspirational and forward thinking health care professional. You should have extensive experience in Care of the older person as well as having excellent leadership, management and communication skills.
Enthusiasm, a desire to learn and flexibility are essential for this role, as are good interpersonal and organisational skills. You will take pride in your responsibility for the quality of the care in your clinical area, provide sound clinical and professional leadership to your multi-disciplinary team and create a culture where the team feel empowered.   1. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the Care of the Elderly team to meet the needs of patients, and will play a key role in service delivery and adherence to policy and procedures. They will also be instrumental in guiding junior medical and nursing staff in relation to relevant protocols and procedures.
  2. The post holder will be required to work within all areas where Care of the Elderly patients may be housed. This includes the medical admissions unit (MAU), Emergency Department (ED), Care of the Elderly wards, outlying and rehabilitation wards undertaking primary and secondary assessments alongside medical colleagues, providing support and guidance to nursing and medical staff in the management of Care of the Elderly patients.
  3. They will work alongside medical, nursing and multi-disciplinary colleagues in the management of Care of the Elderly patients providing support, guidance and advice as required.
    Our ACP team is based between both hospital sites at UHCW and Rugby St. Cross.

COMMUNICATION:

Document clearly all aspects of assessment, diagnosis and management plan clearly and present the findings to the Consultant for Care of the Elderly or Acute Medicine.
Use the SBAR approach to communicate clinical concerns to the senior medical staff in the event that a patient is found to be critically unwell or deteriorating clinically
Communicate complex information effectively to patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
Maintain effective communication with the multidisciplinary team both within Care of the Elderly and with other departments
Act as an advocate for patients and colleagues
Ensure awareness of sources of support and guidance (eg PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate
Delivering a quality service
Ensure own and others clinical practise is research based
Prioritise, organise and manage own workload in a manner that maintains and promotes quality
Critically evaluate and review innovations and developments that are relevant to the area of work
Demonstrate continual evaluation of practice within the defined specialist area, making agreed changes where appropriate.
Deliver care according to local and national guidelines ensure delivery of evidence-based care
Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities
Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
Evaluate patients’ response to health care provision and the effectiveness of care
Continuously assess and monitor risk in their own and others practice and challenge about wider risk factors
Use a structured framework (eg root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents and near-miss events
