Description:

HOW YOU’LL MAKE A DIFFERENCE

As a member of the Preparing for Adulthood team (formally known as the Transition Team) which sits within the 0-25 Disability Service, you will be working with young people with a disability i.e. a learning and/or physical disability from age 13 -25 who have had a Statement of Educational Needs, a current Education Health and Care Plan (EHCP) and or an allocated Social Worker in preparing the young person for adulthood.
This post is for a LTC until 31st March 2025.

WHAT WE NEED FROM YOU

  • You will either be qualified e.g. NVQ 3 or equivalent or have considerable experience of working in similar service areas, with customers who have complex needs.
  • Demonstrate knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.
  • Have good communication and relationship building skills also good report writing skills.
  • You will be able to carry out risk assessments.
  • The team operates from the Kingswood Centre, but you will be expected to work within the community so a driving licence is essential.
    Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.

Responsibilities:

  • Working in an outreach strength based model, your role is to promote the independence of Children and Young People with the aim of enabling them to reach their full potential both within their home and in the wider community.
  • You will facilitate and enable children/young people in partnership with their families/ carers, key organisations and providers both statutory and voluntary, to find solutions within the community to build resilience and support Local First.
  • Working across the 4 key Preparing for Adulthood pathways you will help young people through their transition to:
  • Gain skills and prepare for further education, employment, training or volunteer work
  • Help being part of the persons local community and building relationships
  • Guidance and support in accessing and maintaining housing
  • Guidance in staying safe, healthy and connection into health services where appropriate
  • You will work alongside other professional such as the SEND team, Colleges, Social care and health professionals.
