QUALIFICATIONS :

• Diplôme collégial ou universitaire en santé publique ou un autre domaine en santé et services sociaux obtenu d’une institution reconnue
• Détenir un certificat sur l’offre des soins prénataux et postnataux (ou être disposé (e) à suivre cette formation)
• Excellente connaissance du français oral et écrit, bonne connaissance de l’anglais
• Deux à trois années d’expérience en santé pulique ou dans le domaine de santé/services sociaux
• Expérience en recherche et planification, en promotion de la santé et gestion d’activités et de projets
• Excellentes capacités de communication (incluant l’aisance à animer des groupes et présenter des ateliers), de relations interpersonnelles et de résolution de problèmes
• Connaissance des problématiques reliées aux déterminants de la santé
• Engagement envers les interventions axées sur la collectivité
• Connaissance des ressources communautaires appropriées au programme
• Connaissances approfondies des besoins spéciaux des nouvelles mères
• Aptitude à travailler avec des mères enceintes, des pères, et des nouveau-nés
• Faire preuve d’initiative et être capable de travailler d’une façon autonome
• Être disposé(e) à travailler selon un horaire variable y inclue des soirées pour répondre aux besoins de la clientèle
• Posséder un permis de conduire valide de l’Ontario et avoir accès à un véhicule automobile
• Être confortable à effectuer des visites au domicile des clients
• Fournir une vérification d’antécédents judiciaires en vue d’un travail auprès de personnes vulnérables
• Une preuve de vaccination contre la COVID-19 ( ou un billet médical pour une exemption) sera demandée au moment de l’embauche

QUALIFICATIONS:

• College or university degree in public health or other health related and social services field from a recognized institution
• Hold a Certificate for the provision of prenatal and postnatal care (or willingness to take this training)
• Excellent knowledge of oral and written French and good knowledge of English
• Two to three years experience in public health or healthcare/social services field
• Experience in research and planning, health promotion and activity/project management
• Excellent communication (including facilitation and workshop presentation skills), interpersonal and problem-solving skills
• Excellent knowledge of issues related to the determinants of health
• Commitment to community-based interventions
• Knowledge of community resources appropriate to the program
• In-depth knowledge of the special needs of new mothers
• Ability to work with pregnant mothers, fathers, and newborns
• Demonstrate initiative and ability to work independently
• Willingness to work flexible hours including evenings to meet client needs
• Comfortable making home visits to clients
• A valid First Aid & CPR certificate would be an asset
• Have a valid driver’s license and use of a vehicle with proof of appropriate insurance
• Provide a clean Police Vulnerable Person Criminal Record Check (PVSC)
• Proof of COVID-19 vaccination (or medical ticket for exemption) will be requested at the time of hiring

• Provide individual prenatal and postnatal counseling sessions related to maternal health care
• Work in collaboration and consultation with CSCHN medical staff to serve clients enrolled in the program
• Contribute to improve the global health and well-being of families with babies
• Create and promote a support system for new mothers and their newborns
• Promote breastfeeding and its duration by providing prenatal and postnatal support
• Plan, organize and offer group activities, such as prenatal classes, postnatal educational groups, parenting groups
• Provide postnatal follow-up on nutritional care for newborns
• Prepare snacks and cold dishes for group activities
• Conducting home visits and individual consultations
• Plan and prepare materials to teach and demonstrate selected elements during a visit
• Encourage the development and maintenance of strong relationships between parents and children
• Maintain and prepare reports, statistics and prenatal/postnatal records
• Participate in relevant networks/committees as requested by the supervisor
