Description:

MAIN DUTIES The successful applicant will work as part of the frailty MDT for patients on the frailty pathway, in ED,ambulatory care and the acute medical wards contributing to their Comprehensive Geriatric Assessment, therapy input and Consultant Geriatrician support. This will include the initial assessment of patients, organising relevant investigations in an outpatient setting and arranging a supportive discharge including liaising with the virtual ward.Depending on the development of the service the role may develop during the year to become more integrated with opportunities to join the enhanced virtual ward and community teams. The successful applicant can expect to develop skills in the assessment and care of older people in particular common presentations such as falls, delirium, polypharmacy and decreased mobility. The post holder will ensure evidence-based practice is inherent throughout all aspects of care delivery and treatment and will ensure this knowledge is disseminated via robust plans.
Experience Required: Minimum requirement to have completed F2 training program with at least four months previous experience in either Emergency Medicine, General Practice, Geriatric Medicine or Acute Medicine - wishing to gain further experience in an exciting department with a varied and challenging case mix. Previous rotations including geriatric medicine is desirable but not essential. Research interests and involvement in audit projects will be encouraged.
The frailty pathway is a QI project in itself so the successful applicant will receive opportunities for QI training. There are regular weekly and monthly meetings held in the hospitals Postgraduate Centre. Programmes are regularly circulated. In addition there is weekly COOP departmental teaching and insitu simulation training which the successful applicant can be involved with.
The Ambulatory Emergency Care Consultants are committed to the supervision and development of this post. All post holders will have a Consultant allocated as their Educational Supervisor for the duration of the post. To screen for patients in the Emergency and Acute wards who meet the criteria to be seen by the frailty service and to proactively identify patients for the service. To see and treat patients as they present to the department on the frailty pathway and commence comprehensive geriatric assessment/frailty reviews.
To liaise with the rest of the MDT to complete the assessment and plan safe discharge To liaise with the community teams and GPs to maintain and ensure continuity of care and appropriate follow-up of patients To liaise with appropriate specialty teams where necessary to optimize care for patients. To liaise with the imaging department to arrange urgent diagnostics. To actively monitor and help manage patients queues in conjunction with the senior nurse and HCA To undertake, in conjunction with the Consultants, daily review of investigation results returning to the department with the arrangement of patient follow up, when necessary. To work closely with the Emergency Department, Urgent Care Centre and Medical Assessment Units and the COOP Department To participate in the weekly Multi-Disciplinary Team meetings OTHER DUTIES To take part in the formal and informal teaching program of the department including the teaching of nursing and AHP as appropriate.
To maintain his/her own continuing education, attending relevant courses where necessary. To participate in departmental audit and research. The postholder will work 40 hours per week. With usual working hours being 09:00-17:00.
Working less than full time can also be supported. Candidates will be advised of the outcome within three weeks of the closing date To take part in the formal and informal teaching programme of the department including the teaching of nursing and AHP as appropriate. To maintain his/her own continuing education, attending relevant courses where necessary. To participate in departmental audit and research.
To participate in insitu simulation training and help deliver it once competent The postholder will be expected to work 40 hours per week. With usual working hours being 09:00-17:00. Part time applications as well as a six month contract will be considered
