Description:

The post holder will triage the referrals by telephone following the agreed Standard Operating Procedure and will be based within the triage and referral hub. The triage post holder will work with a see and Treat model that would enable the patient to remain well at home and avoid admission to hospital. Will ensure the patient is assessed by the appropriate professional within the team and coordinate the ongoing clinical and social care reviews in order to support the agreed Key Performance Indicators and ensure patient flow through the system is optimised. If appropriate will signpost the referrer or patient to appropriate alternative services.
The post holder will coordinate the daily care and therapy support visits required by the patient. Adhere to Professional Codes of Practice. Within a Crisis situation offer appropriate holistic assessment intervention, and review to meet their needs. The post holder has continuing daily responsibility and accountability with supervision of the support staff within UCR.
To assess the environment/social situation and identify any risk that could potentially lead to hospital admission. Assesses, plan, implement and reviews routine through to complex nursing care plans using research and evidence-based knowledge to underpin practice and which support avoiding a hospital admission. To supervise, lead, inform and delegate staff as appropriate to the needs of the service by utilising skills and resources effectively as directed by the Urgent Community Response Team Leader. To be an expert resource within the team regarding all aspects of nursing care.
Applies Specialist, analytical and judgemental skills to assess and interpret acute, chronic and palliative conditions, taking appropriate actions to prioritise patients needs. Demonstrates a high level of intuitive and creative problem-solving skills to facilitate patient centred care; rationalising information to create patient concordance by using a high level of communication/negotiation skills to effect the outcome. Participates in the Performance and Development Review process within the team and undertakes review of junior staff under supervision of the manager. To participate in the investigation of complaints and safeguarding concerning nursing services in accordance with the level of experience and encourage comments and suggestions on improving the quality of the service.
To ensure that accidents, complaints and unusual incidents are reported in accordance with the policy. To analyse workloads and constantly review work patterns as required. To participate in systems of clinical governance to achieve quality improvements i.e. clinical audit and evidence-based practice and make recommendations to the team leader for changes to practice.
