Description:

An exciting opportunity is available within our Urgent Community Response Team for a Band 6 Co-ordinator.
The post holder will work in a Multi-disciplinary team of Senior Nurses, Advanced Clinical Practitioners, Senior therapists and Support Workers and play a pivotal role in the triaging and acceptance of referrals into the team.
The post holder needs to have excellent communication and problem-solving skills to facilitate best patient centred care.
Urgent Community Response is a service operating a multidisciplinary approach, providing wrap around holistic care to patients in their own home for 72 hrs and so preventing ED attendance and avoidable hospital admissions. The team is made up of Senior Nurses, ACP’s, Senior Therapists and Support Workers.
The Urgent Community Response Triage process is based on the Manchester Triage Assessment Tool. This role is to triage referrals, ensuring the patient is allocated to the appropriate member of the multidisciplinary team or signposted to the appropriate service. This role is pivotal to the “see and treat” ethos of the team as patients are assessed within 2 hours of referral.
The post also entails supporting the allocated professional to coordinate the patient’s holistic intervention whilst ensuring appropriate onward referrals are made to maintain patient flow through the service. There will be a clinical aspect in order to prevent avoidable hospital admissions.
