Description:

“This is an excellent opportunity to join our team as a UCR and Hospital Discharge Service Clinical Co-ordinator. Covering a fixed term maternity leave for 12 months
We are looking for a Registered Nurse or Allied Health Professional, to work with our community teams providing clinical triage in order to facilitate timely discharge from an acute hospital stay or to remain safely at home when facing an unexpected health crisis. The role includes integrated MDT working targeted at people living with frailty, multi-morbidity and/or complex needs to help them stay independent and healthy for as long as possible.
There will be an excellent induction programme, regular clinical supervision support, alongside opportunity for professional development.
To provide high quality and timely clinical triage for referrals coming in to the service, providing advice and guidance to referrers and liaising with internal teams and partner organisations to support patient care delivery
To be an integral part of an MDT consisting of Nurses, Physiotherapists, Occupational therapists and Support workers delivering patient centred care to maximise independence, confidence and ability.
To demonstrate specialist clinical skills in practice for patients in need of UCR or Intermediate Care support
To provide support/advice regarding service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies.”
