Description:

A. Planning and Organisational Support the development of Urology Cancer Pathway across NWAFT including the triage, support management of 2WW appointments and identify streamlining opportunities by working alongside colleagues in Diagnostic Imaging and Pathology. To explore current practice, identifying areas for development and planning appropriate change using current evidence to evaluate outcomes. Work closely with FDS Navigator to mitigate patients diagnosed over 28 days.
This role will be a key stakeholder in pathway development of Urology cancer pathway across NWAFT. The role will work closely with all stakeholders involved in the Urology Cancer Pathways to find solutions, drive collaborative working and ensure continuous improvement. Working with, but not limited to the below. Imaging, Radiology, Pathology Clinical Nurse Specialists CBU Operational team CBU admin team Cancer FDS Navigator Cancer tracker Consultants/ Clinicians Cancer performance team B.
Management Liaise closely with Operation management in the CBU, Division and wider Trust working toward common goals to improve FDS standard. Prioritise own workload and manage time effectively when working alone or in collaboration with others. Manage the work by identifying priorities and developing supportive networks. Support of development pathways between Northwest Anglia Foundation Trust and tertiary Hospitals.
Work with colleagues in implementing local and national guidelines. Act as a role model for others. Be accountable for own actions and professional conduct. Contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
Contribute to service planning and delivery, attending annual general meetings, planning year forward work plans and undertaking patient surveys to inform service development. Visualise and map patient pathways by reviewing the last 10 patient breaches with the support of the cancer management team, reviewing delays to understand trends or common underlying reasons for delays and identify areas for improvement. C. Communication responsibilities Promote and review the use of cancer-related patient information, ensuring that all documents meet corporate standards and follow local, Network and National guidelines.
Assist in facilitating the development of communication with patients and user involvement in services, including patient support groups. Provides extensive support to patients and carers utilising advanced communication skills when patients are exposed to distressing and emotional circumstances. D. Educational Optimise every opportunity to maintain and improve professional knowledge skills and competence.
To educate and provide specialist advice to the multi-disciplinary team. Support, initiate and maintain close liaison with the multi-disciplinary team within the hospital and the primary care team. Participate in annual MPA reviews. Attend mandatory training in accordance with Trust policy and adhere to all Trust Policies and Procedures.
o Maintain a level of practice in accordance with hospital policy, NMC Code of Professional Conduct and Scope of Professional Practice in order to meet the changing needs of the patient and service. o Participate in clinical supervision. Keep up to date with research and update practice accordingly. To use and participate in appropriate networks to develop and enhance practice.
Support the education of nursing and medical students in the department. E. Personal Development Contribute to the development of specialist guidelines and protocols in collaboration with colleagues to optimise the care of patients on a suspected Urology Cancers. Develop collaborative communication pathways across service providers, including primary, secondary, and tertiary care settings, to contribute to future strategic planning and development of cancer services.
Act as a role model to ensure high standards throughout the patient pathway, practicing within the clinical governance framework, promoting a learning environment for professional development and being accountable for continuously improving the quality of our services and safeguarding high standards of care where excellent clinical care already exist. F. Service and Policy Development Develop and review protocols, policies and procedures, in conjunction with the Lead Nurse and Matron, to promote safe, equitable and consistent care for all Urology cancer patients. Contribute to the development of specialist guidelines and protocols in collaboration.
with colleagues to optimise the care of patients on a suspected Urology Cancer pathways Work with other members of the Clinical Urology MDT to develop mechanisms for monitoring and maintaining care in relation to national and local cancer standards. Maintain records of clinical service, including developing methods of evaluation and monitoring patient progress, including audit. Establish and maintain rolling audit programmes relating to Urology Cancers, in particular those relating to NICE Guidelines. Promote innovation in developing care for cancer patients.
Undertake patient surveys in order to develop and inform services. G. Research and Development (includes Risk & Governance Management) Participate and undertake clinical audit, as appropriate to the role and identify areas for audit and evaluation in the management of cancer patients and future practice. Attend audit meetings on a regular basis and be willing to present findings as appropriate.
Keep abreast of research developments within oncology. Develop and update patient information as necessary
