Description:

The Urostomy and Bladder Practitioner will form an integral part of the multidisciplinary team in caring for the physical, social and psychological needs of patients and carers diagnosed with Bladder cancer throughout their treatment pathway but with special focus on urostomy care.
The post holder will have daily direct contact with patients and their families, providing teaching, support, advice and signposting. The Role is unique and imperative to patient care after Cystectomy.
The role will also involve the undertaking of administrative and clinical tasks deemed suitable to the role by the Clinical Nurse Specialists (CNS’s).
The post holder will work as part of the Urology multidisciplinary team under the guidance and supervision of a registered practitioner.

The role will be teach Urostomy care to the patients .
To support patients and be a direct point of contact for troubleshooting troublesome urostomy
Triaging telephone calls and providing basic advice and support.
Liaising and referring to other appropriate healthcare professionals especially within the community
Arranging and supporting clinics with BCG
Arranging and triaging of the electronic Holistic Needs Assessments (eHNA) during the cancer patient pathway. This will include the navigator assisting the patient with completing their eHNA where appropriate. This will ensure that patients are signposted in a timely manner to appropriate services.
Ensure the appropriate management of patient information and the co-ordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical nurse specialists and other departments as required within the Trust
To work with the CNS’s to develop and embed the Living With and Beyond Cancer agenda within the Urology service.

Data Collection and Analysis
Assist in maintaining and developing the Urology CNS patient database, including remote monitoring
Work closely with the MDT co-ordinator to ensure roles are well defined and not duplicate
Quality Issues
Assist in the development of care pathways and implement improvements to the administrative element of the pathwa
Ensure that directorate administrative and clerical functions are carried out to the highest quality standards by monitoring and improving services
Monitor patients’ progress through the cancer pathway
Identify areas where targets are not achieved and undertake process mapping to identify bottlenecks and improve performance.

To participate and display a willingness to learn develop a relevant technical competency/area of interest and keep up to date with current local and national issues and developments
To be responsible for own learning in knowledge base relevant to own clinical area including mandatory training
To participate in regular formal supervision and develop a personal development plan that links into KSF and departmental competency frameworks in order to promote learning and enhance skills
