Description:
YOUR SKILLS AND EXPERIENCE:
- Current registration as a Registered Nurse
- Demonstrated commitment to ongoing personal and professional development, as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio (PPP)
- Demonstrated capacity and skill in coordinating care for chronically ill patients and/or vulnerable children
- Demonstrated capacity to work constructively and collaboratively with a range of disciplines towards common goals
- Proven ability and skill to build collaborative relationships and work effectively within multidisciplinary teams
- Post Graduate qualification or working towards

Responsibilities:
This is a Permanent, Full Time position with at 76 hours per fortnight within the Diabetes Allied Health Team.
You will work in conjunction with the Diabetes and Allied Health team, and be responsible for identifying and coordinating care and support for the most vulnerable patients within the Diabeties service at the Royal Childrems Hospital (RCH). Thsi role will be involved in identifying and addressing psychosocial concerns, determining appropriate plans of support, referral to community bases services, and liaison with key organisations such as statutory child protection services.
Classification for this position will range from ZF4 - ZJ4, Clinical Nurse Consultant A - Clinicial Nurse Consultant B, (FTE base salary $107,962 to $113,646 per annum, plus superannuation)
