    Signature Care presents an exciting opportunity to shape the service delivery of aged care into the future during a period of significant change.
    We are currently seeking a permanent part time experienced and qualified Clinical Care Coordinator to work closely with our Clinical Manager and Residential Manager to develop our stunning 144-bed aged care residence based in Tamworth.
    The Clinical Care Coordinator (CCC) will provide weekend clinical leadership to the team, foster a culture of clinical excellence and oversee the clinical care of our residents to ensure that a high standard of care is maintained.

Skills and requirements for this role:

  • Ideally at least 2+ years experience as a Clinical Care Coordinator or similar role in the aged care or acute sector;
  • RN qualification with current Ahpra registration;
  • Excellent and up to date clinical knowledge and assessment skills;
  • Experience in clinical oversight, staff supervision and development;
  • Strong communication and negotiation skills;
  • A working knowledge of funding and accreditation processes;
  • Knowledge of Care Planning.
