Description:

KNOWLEDGE/EXPERIENCE:

  • Bachelor’s Degree in social work, consulting, or similar field of study required.
  • Two years of experience in working with children and families.

SKILLS / ABILITIES:

  • An ability to work well with youth who have been diagnosed as having a serious emotional disturbance. An ability to use a positive, strength-based approach and be able to relate warmly to families and children. Ability to utilize the wraparound model to facilitate change in youth and families.
  • An ability to put the needs of the youth and the family first and the ability to teach families how to respond appropriately in a crisis.
  • An ability to follow all written and verbal instructions, and to communicate with all programs staff.
  • Ability to work as a team player, including viewing the family members as capable decision makers.
  • An ability to implement high fidelity wraparound principals and facilitate the child and family team meetings.
  • Must have the ability to relate to a wide diversity of people in a compassionate, and non-judgmental manner.
  • Good organizational and time management skills. Must be able to work independently and meet assigned deadlines.
  • Ability to be flexible.

PHYSICAL/MENTAL REQUIREMENTS:

  • Must be able to read, write, and speak English.
  • Manual dexterity and motor skills required in completing documentation and other duties as required.
  • Must be able to drive and have access to a vehicle day and night.
  • Demonstrate maturity and flexibility in dealing with emotionally demanding and stressful situations.
  • Must be emotionally stable, alert, and able to work alone, and be self-motivated.

Responsibilities:

  • The Wraparound Facilitator will maintain a caseload of 10 wraparound cases which includes all identified youth and families who may not utilize residential services but need short-term intensive community support.
  • The Wraparound Facilitator will be responsible for creating a Plan of Care for all families who have children receiving Wraparound services.
  • The Wraparound Facilitator will maintain accurate documentation on all activities in a timely manner. This includes but is not limited to post it sheets for meetings, case notes, plan of care, case staffing notes, and weekly/monthly reports.
  • The Wraparound Facilitator will maintain confidentiality both in documenting signed release of information forms and in day-to-day wraparound interactions with community resources, youth and families, and formal/informal supports. This will include obtaining information from other child serving agencies involved with the consumers served.
  • Will work with the Therapist to meet the needs of the youth and family and to facilitate the child and family team meetings which encompasses the family and their team.
  • The Wraparound Facilitator will develop and maintain contacts with formal and informal supports to better facilitate the Child and Family Team Meetings.
  • The Wraparound Facilitator will provide transportation as needed for the families or will decide for the families’ transportation to appointments.
  • The Wraparound Facilitator will create a plan of care that provides information and guidance to the family to help them meet their needs pre/post wraparound services.
  • The Wraparound Facilitator will be responsible for building teams and facilitating the Child and Family Team Meetings.
  • Wraparound Facilitator will ensure that each family receives the minimum contacts with youth(s), family members, formal/informal supports, and the assigned Therapist.
  • All other duties as assigned.
