Description:

Responsibilities:

  • Provide outreach services to youth incarcerated at the Calgary Young Offender Centre
  • Provide release planning and ongoing case management support in the community for youth as needed
  • Help address immediate needs that address barriers to integration/social inclusion
  • Accompany and transport youth to access resources in the community and attend appointments
  • Transport youth in your personal vehicle with compensation for mileage
  • Liaise regularly with client support systems (AHS – addictions/mental health workers, institution caseworkers, internal programming staff, community corrections staff, family, employer, landlord, etc.)
  • Follow up with youth to assess progress towards goal and/or goal attainment
  • Program marketing, networking, and building supportive working relationships with community resources
  • Ongoing development of intake and assessment processes, outcome measurement and evaluation tools
  • Data collection, database input, casework documentation, participant evaluations and administrative tasks
  • Internal and external reporting as required
  • Participate in regularly scheduled agency and team meeting
