Description:
Responsibilities:
- Provide outreach services to youth incarcerated at the Calgary Young Offender Centre
- Provide release planning and ongoing case management support in the community for youth as needed
- Help address immediate needs that address barriers to integration/social inclusion
- Accompany and transport youth to access resources in the community and attend appointments
- Transport youth in your personal vehicle with compensation for mileage
- Liaise regularly with client support systems (AHS – addictions/mental health workers, institution caseworkers, internal programming staff, community corrections staff, family, employer, landlord, etc.)
- Follow up with youth to assess progress towards goal and/or goal attainment
- Program marketing, networking, and building supportive working relationships with community resources
- Ongoing development of intake and assessment processes, outcome measurement and evaluation tools
- Data collection, database input, casework documentation, participant evaluations and administrative tasks
- Internal and external reporting as required
- Participate in regularly scheduled agency and team meeting
