Classification: Grade 2 Social Worker (SC21 – SC24) Salary: $90,235 – $103,724 per annum, pro rata Key Responsibilities Undertake comprehensive psychosocial assessments and formulate appropriate intervention and discharge plans. Apply advanced clinical reasoning to support patients with complex psychosocial needs. Liaise effectively with patients, families, carers and multidisciplinary team members to coordinate care and facilitate positive outcomes. Support safe discharge planning and continuity of care. Participate in quality improvement, service development and research activities. Provide supervision and contribute to student and clinical education programs as required.
