Description

An opportunity has arisen for dynamic and motivated Band 5 Public Health Staff Nurse’s to work within the newly integrated 0-19 Children’s and Young People Public Health Service in the North East Locality based in Ambrosden.
This includes Bicester and the surrounding areas. There is an expectation to work over the whole locality. The new service brings together Health Visiting, School Health Nursing, and the Family Nurse Partnership, delivering the Healthy Child Programme (HCP) to provide an innovative and high-quality service that helps children, young people and families to thrive.
Health visitors, community public health nurses (previously known as community staff nurses) and community public health associates (previously known as community nursery nurses) will now be working with children up to 8 years of age where there are known concerns or vulnerabilities and will provide health related episodes of care to support with areas such as behaviour, diet, and sleep. There has also been an introduction of a new four-year health and development review to support school transition and address any school readiness concerns.
The role offers the opportunity to engage in workstreams within the service to support professional and leadership development. The role has the opportunity to deliver community-based health promotion activities as well as supporting Oxfordshire’s Young Parents Pathway alongside Specialist Practitioners.
Within the caseload, and as delegated by the Specialist Community Public Health Nurse the post holder will contribute to the safe and effective delivery of the 0-19 service.
You will be based within the locality (neighbourhood) and will work with the 0-19 teams in that locality and supporting county wide work where needed for children aged 0-8 years. The role involves working in partnership with children, young people, families, primary care, schools and statutory and voluntary services and other partner agencies to facilitate health-enhancing activities and improve health outcomes.
You will deliver services to children and families within homes, and community settings.
Clinical activities will include assessing, planning, implementing and evaluating family focused care under the delegation of a SCPHN Health Visitor/School Health Nurse and working in partnership with other agencies.
The role includes undertaking Healthy Child Programme developmental and transitional assessments and planning, delivering and evaluating public health initiatives through community-based health promotion activities.
The role will require the post holder to adhere to Safeguarding policies and actively engage under the supervision from a SCPHN in safeguarding processes, including Early help assessments and Team around the Family processes.
You’ll be working in partnership with other agencies and develop, participate and maintain interagency networks.
