The role aims to deliver high quality person-centered, goal-orientated care within a supportive and specialist multidisciplinary medical and allied health team. The role focuses on meeting the functional needs of patients living with the late effects of Poliomyelitis encompassing the person’s environment, occupations and assistive technology needs. Essential to work Wednesdays and Thursdays including opportunity to participate in regional clinics with the Polio Services Team. Provision of Occupational Therapy assessments, management, recommendations and ongoing referrals for clients presenting to Polio Services Victoria. Provide specialist assessments in conjunction with Physiotherapy in the area of posture and seating with Grade 3 Support. Ensure provision is timely and to a high clinical standard, utilising accepted best practice and innovative techniques as appropriate. Work collaboratively with the Polio Services team to provide ongoing education and advice to clients and health professionals in regard to the Occupational Therapy management of people with a history of polio. Work with the Polio Services team in research and service development to ensure the service meets the needs of clients and is implementing accepted best practice. The role requires supervision of student placements in conjunction with other HIP services. Contact Denise Currie denise.currie@svha.org.au LOCAL WORK ENVIRONMENT The Health Independence Program (HIP) was introduced by the DH&HS in 2008 bringing together the four previously separate service streams of Subacute Ambulatory Care, Post-Acute Care, Hospital Admission Risk Program and Residential In Reach under common guidelines. HIP consists of integrated hub sites located across several locations including in Fitzroy, Kew, North Richmond and Northcote. The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning. This position may be required to work at any of the HIP hub sites / programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training. Within the HIP Disability and Pain Stream, Polio Services Victoria (PSV) delivers a multidisciplinary outpatient service from St. Vincent’s Fitzroy campus, with six mandated regional service delivery clinics in Victoria annually. PSV was established by the Victorian Government in 1998 to provide ongoing expertise and support for people with a history of contracting polio. This includes specialist multidisciplinary assessment, provision of information and education to health providers and clients, liaison with other health care providers and clients to develop patient-centred plans and delivery of polio specific specialist care. Clinicians are required to work across a number ofseveral settings as part of the continuum of care. This may include a client’s residence, community settings, the HIP Clinics and regional Victorian clinics. Services may be delivered in a group or individual setting. POSITION PURPOSE To provide evidenced based, high quality occupational therapy services for clients of Polio Services Victoria (PSV). Clinicians are required to provide assessment and management of clients in a number of settings across the continuum of care within HIP, including client’s residence, community settings, Specialist Clinics, Regional Clinics and/or by Telehealth.
