Develop individualized treatment plans and coordinate specialty referrals. Collaborate with physicians, nursing staff, behavioral health providers, and interdisciplinary care teams. Utilize dedicated administrative time for documentation, care coordination, and patient follow-up. Serve a diverse and underserved patient population in a mission-driven community health setting. WHY NURSE PRACTITIONERS ARE APPLYING Monday-Friday schedule with one rotating late clinic day. Outpatient-only primary care practice. 18-24 patients per day. Established team-based care model. Loan repayment eligible opportunity. Four weeks of PTO plus paid holidays, sick time, and personal time.
