SUMMARY:
The Community Health Worker 1 (Patient Navigator) will report to the department of Preventive Health Services and collaborate with the patient’s health plan to enhance the member experience for all lives assigned to the Watts Healthcare Corporation (WHCC). The role is multi-disciplinary in nature and will collaborate very closely with staff of the clinic operations, clinical, member services and quality teams as needed. This position will contribute as a team member to facilitate and enhance access to, appropriate utilization of, and satisfaction with Watts Healthcare Corporation’s services by patients and community members. The Community Health Worker 1, is responsible for connecting patients with their primary care provider for appointments, and providing in-person Community Supports, Asthma Remediation Services in conjunction with culturally appropriate asthma self-management education, engagement and trigger remediation services.
